When will my plan take effect? The plan goes into effect the 1st day of the second month following receipt of application.

Can I really go to any dentist? Yes, benefits are payable to any licensed dental provider. Dentists can call Ameritas prior to treatment to verify how the claim will be paid.

Can I participate in this plan along with the dental plan I currently have? Yes, this plan will coordinate benefits payable with other insurance plans.

How do I find a Vision Service Plan (VSP) provider for the Routine Vision Exam? You can visit the VSP Internet site at www.vsp.com and click on the link that says “Considering Purchasing VSP?”

What day will the premium be taken out of my checking account if I choose the PAID option? The money will be taken out of your account the 10^th of every month for the following month’s premium.
