Q: How Do I Enroll For Coverage?
Q: When Does Coverage Begin?
Answer: Coverage will be effective on the first of the month
following approval of your application and receipt of your first premium,
provided you are on active service at that time. "Active Service" means that you
are able to perform all of the regular duties of your job, regardless of whether
or not you are scheduled to work that day. Coverage is not effective on the date
of your application. The effective date for your eligible dependents will be the
same as yours (unless dependent coverage is added at a later time). However, if
any dependent is totally disabled on their effective date, the coverage of that
dependent will be deferred until the dependent ceases to be disabled. A
dependent is totally disabled if unable to perform a majority of the normal
activities of a person of like age in good health.

Q: When Will My Coverage End?
Answer: For you, coverage will end the first of the month after
we receive a written request to terminate your coverage (when you no longer
qualify as an eligible insured you must provide us this information in writing
for us to cancel your policy), , or if your premium payments are discontinued,
whichever comes first. Coverage on a dependent ends on the earlier of the date
that they no longer meet the definition of a dependent, or the date that your
coverage terminates (both of these situations need to be sent to our office in
writing).

Q: Who is Eligible For Coverage?
Answer: All active members of an approved association are
eligible. Dependent coverage is available to a married spouse and any unmarried
children or stepchildren, under age 19, who are living with the insured.
Full-time students going to an accredited school and under age 24, are also
eligible for coverage.

Q: Can I Sign Up For
Coverage At Any Time?
Answer: No. If you do not enroll during your initial
eligibility period, you cannot enroll until your association's next open
enrollment period. You may add dependent coverage when you enroll, during an
open enrollment period, or within 31 days of a family status change.

Q: Can I Cancel My Coverage
At Any Time?
Answer: Yes. You can drop coverage at any time. However
- this must be done in writing to Greater Insurance Service and the earliest
possible cancellation date will be the first of the month after your
cancellation request is received.

Q: How Much Does It Cost?
Answer: At the cost of about one or two hours of work per week,
you can have health care coverage and peace of mind 24 hours a day, seven days a
week. And for just a little bit more, you can cover your eligible family
members.

Q: How Do I Enroll for Coverage?
Answer: You may enroll for your benefits when you first become
eligible or during one of your association's open enrollment periods. Enrolling
for your benefits is as easy as 1,2,3.
