Q: When Does Coverage Begin?

Answer: Coverage will be effective on the first of the month following receipt of your enrollment form, payment option form and your first month's premium. The effective date for your eligible dependents will be the same as yours (unless dependent coverage is added at a later time).

Q: When Will My Coverage End?

Answer: For you, coverage will end the first of the month after we receive a written request to terminate your coverage (when you no longer qualify as an eligible insured you must provide us this information in writing for us to cancel your policy), or if your premium payments are discontinued, whichever comes first. Coverage on a dependent ends on the earlier of the date that they no longer meet the definition of a dependent, or the date that your coverage terminates (both of these situations need to be sent to our office in writing).
